By integrating transplant-trained clinicians, 24/7 clinical support, in-home care, and seamless electronic health record integration, OPT aims to help centers deliver high-touch, scalable care that reduces transplant patient readmissions, enhances patient adherence, and supports transplant volume growth. Susan is committed to fostering collaboration among transplant centers, clinicians, and patients to ensure that care is optimized, personalized, and sustainable.
